Medication Information and Forms
Our clinics must have a Medication Permission Slip on file for students requiring medication administered during school hours. The Over the Counter Medication Form must be signed by a parent/legal guardian. Your child's physician must complete and sign the Prescription Medication Form.
Medication must be kept in the ORIGINAL bottle.
A parent or legal guardian must drop off and pick up leftover medication at the nurse's office. Students are not permitted to carry medication with them in school or on the bus.
